Artificial Intelligence in Social Services: Predictive Analysis and Identification of Intervention Needs

Artificial Intelligence (AI) is arguably the most innovative and relevant tool of recent times. Its possibilities are vast, and in the field of Social Services, it offers significant opportunities to improve the quality and efficiency of services.

AI algorithms allow for the analysis of data from various sources (such as housing records, healthcare databases, and social services usage) to identify patterns and risk factors. Through predictive models, Artificial Intelligence can detect urgent needs, optimize resource allocation, and facilitate early intervention. Moreover, it can adapt to individual needs, offering personalized support tailored to each situation, which greatly enhances the outcomes of social services.

However, despite its advantages, the implementation of AI-based tools in social services also poses challenges such as privacy protection, data security, and the need for training professionals in the sector, all of which are essential to ensure the technology is used ethically and appropriately.

In this Innobreak, we present two pioneering initiatives and best practice examples that incorporate AI to support social intervention:

  • wSocial (Catalonia) is a tool promoted by the Department of Social Rights and based on Artificial Intelligence. By using keywords, it can detect vulnerable situations and propose interventions. Focused on people’s care, it streamlines processes, automates tasks, and provides information for decision-making, planning, evaluation, and transparency. It has already been implemented in the social services of Manresa, Vic, Aran, and Esplugues de Llobregat, and will gradually be rolled out across all Basic Areas of Social Services in Catalonia.
  • Nrativ (Sweden) is an Artificial Intelligence model that automates and optimizes the process of reading and analyzing text-based reports, creating unique categories and visualizing trends over time. This simplifies data review, especially when managing complex information. By applying the Nrativ AI model to its data, the local government of Helsingborg can track and ensure the improvement of its services, with a special focus on people with disabilities and elderly care.

El nuevo piloto aplicará la herramienta NIDUS para acompañar a personas con trastornos mentales graves en el proceso de desinstitucionalización

El proyecto NIDUS Desinstitucionalización ha sido seleccionado en la 2ª Convocatoria del Hub de Innovación Social y Salud (HISS), que este año aborda el reto de las Alternativas Comunitarias a la Institucionalización. Este proyecto constituye una ampliación de la aplicación NIDUS, hasta ahora destinada a personas en situación de precariedad, con el objetivo de adaptarla al ámbito de la salud mental.

El piloto de NIDUS Desinstitucionalización tiene como finalidad facilitar los procesos individuales de transición a la comunidad y el fortalecimiento de los vínculos para prevenir, retrasar o minimizar la institucionalización de personas con trastornos mentales graves, promoviendo su autonomía y calidad de vida bajo los estándares de “Quality Rights”.

El proyecto consistirá en la ampliación de la herramienta NIDUS, desarrollada por iSocial desde 2021 para apoyar a personas en situación vulnerable. Entre las principales funcionalidades de la herramienta digital se incluyen una Caja Fuerte Virtual para la gestión de documentación personal y una plataforma de comunicación directa con los profesionales de referencia, que permite un acompañamiento continuado y personalizado. Además, el piloto incluirá una nueva funcionalidad, Canal 24h, que permitirá, mediante un sencillo cuestionario, conocer el estado de salud, bienestar y posibles riesgos de las personas acompañadas.

La 2ª Convocatoria del HISS se enmarca en el programa Transforma, que tiene como objetivo apoyar proyectos innovadores en los ámbitos de la salud y la atención social, fomentando su transformación y adaptación a los nuevos tiempos. El proyecto NIDUS Desinstitucionalización ha sido seleccionado junto con otras nueve iniciativas y recibirá un acompañamiento específico durante 10 meses para su desarrollo e implementación.

Gracias al apoyo del HISS, se podrá cumplir el objetivo de NIDUS Desinstitucionalización: promover un modelo de atención centrado en la persona que facilite su transición a la comunidad, promoviendo su autonomía y reduciendo la dependencia de los servicios institucionales. Una vez probada la utilidad de la herramienta y su eficacia, se prevé escalar su uso para que pueda ser utilizada por un amplio número de personas afectadas por procesos de desinstitucionalización o en riesgo de institucionalización en Cataluña.

El proyecto cuenta también con la colaboración de Pere Claver Grup, Sant Joan de Déu, el Institut Pere Mata, así como con la consultora Antares Consulting y la empresa Inqbarna, que se encargará del desarrollo de la app.

The new pilot will implement the NIDUS tool to support individuals with severe mental disorders in the deinstitutionalization process

The NIDUS Deinstitutionalization project has been selected in the 2nd Call of the Social and Health Innovation Hub (HISS in its Catalan acronym), which this year focuses on the challenge of Community Alternatives to Institutionalization. This project is an extension of the NIDUS application, which has so far been used to support people in precarious situations, and now aims to adapt it to the mental health field.

The NIDUS Deinstitutionalization pilot seeks to facilitate individual transitions to community life and strengthen ties to prevent, delay, or minimize the institutionalization of people with severe mental disorders, while promoting their autonomy and quality of life, aligned with the “Quality Rights” standards.

The project involves expanding the NIDUS tool, developed by iSocial since 2021 to assist people in vulnerable situations. Key features include a Virtual Safe for managing personal documents and a platform for direct communication with reference professionals, enabling continuous and personalized support. Additionally, the pilot will introduce a new 24-hour Channel, which will use a simple questionnaire to monitor the health, well-being, and potential risks of the supported individuals.

The 2nd HISS Call is part of the Transforma program, which aims to support innovative projects in the health and social care sectors, fostering their transformation and adaptation to current challenges. The NIDUS Deinstitutionalization project was selected alongside nine other initiatives and will receive tailored support over a 10-month period for its development and implementation.

With the backing of HISS, the goal of NIDUS Deinstitutionalization is to promote a person-centered care model that facilitates their transition to community life, enhances their autonomy, and reduces dependence on institutional services. Once the tool’s utility and effectiveness have been validated, it is expected to scale its use, benefiting a broader population in Catalonia affected by deinstitutionalization processes or at risk of institutionalization.

The project also involves collaboration with Pere Claver Group, Sant Joan de Déu, the Institut Pere Mata, as well as the consultancy Antares Consulting and the tech company Inqbarna, which will be responsible for app development.

ASPACE, an organization specialized in supporting people with cerebral palsy, becomes the 19th member of the Foundation

The iSocial Foundation continues to grow, welcoming new entities from the social sector. This month, ASPACE joins as a new member, adding to the 18 entities that are already part of the Foundation. With this new addition, iSocial now has a total of 19 organizations working together to drive innovation and improve social services.

Founded in 1961 as an association by a group of parents of children with cerebral palsy, the ASPACE Catalunya Foundation has been a pioneer in providing care for people with cerebral palsy and multiple disabilities. Since its inception, it has worked to offer highly specialized, comprehensive care in the fields of neurodevelopment, including rehabilitation, education, and family support. Its innovative and interdisciplinary care model has become a benchmark in Catalonia, with the mission of supporting these individuals throughout their entire lives.

ASPACE‘s values of accessibility, innovation, and solidarity are aligned with iSocial’s mission to promote an innovative social sector capable of adapting to the diverse challenges faced by vulnerable groups.

ASPACE Confederation is the third entity to join the iSocial Foundation in 2024. Before the summer holidays, Grupo SSI and REIR (Educational Resources for Children at Risk) also became part of the iSocial project.

At the iSocial Foundation, we are delighted to continue adding expertise in order to pursue our mission of driving innovation in the social sector, fostering cross-sector collaboration and cooperation. We will keep working together to build a stronger, more connected, and inclusive sector that can respond to the needs of those who require it the most.

Montferrer i Castellbò integrates healthcare professionals into the Vincles project

The training is part of a pilot project aimed at integrating the healthcare system into the Vincles project

On September 5th, Montferrer i Castellbò hosted a training session for primary care and Sant Hospital de La Seu d’Urgell healthcare staff who serve the population of this municipality. The goal was to incorporate these professionals and the healthcare system as a whole into Vincles, a project that focuses on detecting and preventing unwanted loneliness.

This initiative is a pilot program led by the iSocial Foundation and the Alt Urgell Care Consortium, supported by the Social and Healthcare Innovation Hub (HISS). It aims to integrate the healthcare system into Vincles, which has been rolled out since last year across six regions of Alt Pirineu and Aran under the leadership of regional Social Services. The goal is to ensure that, in the future, responses to unwanted loneliness among the elderly become a sphere of integrated social and healthcare services across these six regions.

So far, the Vincles Alt Pirineu-Aran project has been driven by the Social Services of the six regions within the Alt Pirineu and Aran territories. In collaboration with them, 53 social and demographic indicators have been established, allowing the identification of potential risks of unwanted loneliness in a geolocated manner through a data analysis tool. Once risk areas are identified, Vincles intervenes by activating the community network and fostering social engagement.

With the pilot launching this week in Montferrer i Castellbò, additional health-related indicators will be incorporated into Vincles, improving accuracy in detecting loneliness risks. Furthermore, healthcare professionals will become an integral part of the support network for individuals experiencing unwanted loneliness in this municipality, both during the detection phase, using the aforementioned health indicators, and during the intervention phase, by prescribing community activities and collaborating with the Social Services teams of Montferrer i Castellbò and the Alt Urgell region.

This pilot represents a significant step in the expansion of the Vincles project and its adaptation to address loneliness from an interdisciplinary perspective, with the ultimate goal of improving the quality of life for people in the Alt Pirineu-Aran territories.

Video calls aimed at carers with the goal of sharing experiences and building community

Cuppa is a free 45-minute virtual meetup program specially designed for unpaid carers. Through video calls, around a dozen carers come together to share their experiences, offer support, and meet others in similar situations in a safe and understanding environment.

The meetings are moderated by a host, also a carer, who facilitates the conversation and ensures everyone has the opportunity to speak. The sessions are simple and accessible, requiring only a device with internet and a microphone. Using the camera is recommended but not essential, and there is also the option to interact via chat.

Additionally, to cater to the varying needs of carers, a wide range of time slots is available, and attendance is unlimited. Cuppa aims not only to provide emotional support to carers but also to create a network of people who share tips and experiences, always with a touch of humor and optimism.

Solidigital’s Second Project Meeting Takes Place in Andorra

The seven partner organizations of the European project gather in Andorra to coordinate Solidigital’s deployment plan

On September 5th and 6th, we held the second meeting of the Solidigital project in Andorra la Vella, with the participation of seven entities from Catalonia, France, and Andorra. The aim of the meeting was to coordinate the first phase of this European project’s deployment, driven within the framework of the European Union’s cross-border collaboration program, Interreg Poctefa.

Solidigital, which aims to facilitate access to social services for people in vulnerable situations, as well as the support work of professionals assisting them, is based on the use of three innovative digital tools: Soliguia, an interactive map of social resources providing detailed information on services available in each region; our app Nidus, a virtual safe box designed to support people in precarious situations; and Reconnect’s Coffre-fort numérique, a similar tool to Nidus operating in France.

During the meeting, we had the opportunity to learn more about these three tools, address integration issues, resolve doubts, generate debate, and, above all, get to know better the team we work with daily. Moreover, thanks to our hosts, Andorra Research and Innovation, we had the chance to meet Laura Mas de la Rosa, Director of the Department of Childhood, Adolescence, and Youth, and visit the facilities of the Andorran Red Cross to gain a deeper understanding of the country’s social ecosystem.

The meeting laid the groundwork for launching the project’s first phase, which will focus on mapping social resources in each region and will be carried out in collaboration with various local social entities and institutions.

The seven entities involved in the project will carry out this task in their respective regions. On the Catalan side, the project will involve the iSocial Foundation, project leader, Resilis, Alba, and Pere Claver, along with Solinum, which will coordinate the creation of Soliguia across all regions. In Andorra, the Foundation Andorra Research and Innovation will be responsible for implementing the project, while in France, Solinum and Reconnect will participate.

The project also includes the interconnection of these three tools and a training plan for 8,000 professionals, aiming to equip them with the necessary skills to use the tools, ensuring efficient implementation and quicker access to social services for the most vulnerable populations.

The ATENEA system uses a buttonless mobile phone with a highly simplified user interface that enables vulnerable groups, such as the elderly or people with disabilities, to manage their basic needs and request social services digitally. The device only requires voice use and operates through a voicebot that combines natural language processing and artificial intelligence to engage in smooth and agile conversations with users.

The main goal is to reduce the digital gap and ensure everyone has access to the necessary technology to improve their quality of life. Additionally, ATENEA seeks to mitigate unwanted loneliness through easy communication with their immediate environment and increase personal safety by requesting assistance in emergencies.

Furthermore, the tool facilitates access to basic goods such as food through online shopping, helps people orient themselves in space and time, and simplifies interaction with social services by scheduling appointments and enabling direct communication with social workers.

The IWI Project (community in Maori) is a pioneering service in Catalonia that offers support to families with underage children to improve their well-being, using the Family Group Conference restorative justice methodology, originally from New Zealand. This methodology is based on dialogue and trust in the abilities of families to find solutions to their problems through the active involvement of their community and support network.

The project is aimed at families who detect difficulties in their family dynamics and wish to address them. IWI offers a safe space where family members can express their needs and, together with the reference people they select, find community solutions to their problems. This process includes individual meetings with family members, the creation of a support community, and a final meeting where actions to be taken are discussed and agreed upon.

After the family meeting, IWI conducts follow-ups to assess progress and strengthen community ties. This follow-up is an opportunity to acknowledge the community’s effort and recognize the positive changes that have taken place. Thus, IWI not only works to resolve immediate issues but also seeks to strengthen family and community relationships in the long term. Additionally, there are plans to establish physical offices in all four provinces to provide a point of reference for families.

NIDUS Mental Health

Adaptation of the NIDUS app as a digital support tool in mental health for the needs of people with severe mental disorders who are transitioning from long-term hospitalization to community life and/or strengthening their ties with the community.

(2024-2025)

NIDUS Deinstitutionalization is a pilot project aimed at addressing the lack of tools focused on promoting the autonomy, empowerment, and life project development of people with mental health issues who are in the process of deinstitutionalization or at risk of institutionalization. To this end, the NIDUS app from the iSocial Foundation, which has so far been successfully used in the field of homelessness care, will be adapted, piloted, and evaluated.

The adaptation of this digital solution to the mental health field, with a focus on the needs of the individuals, will enable:

• A connection and easy communication between the individual and their reference professionals

• Support and facilitation of tools for the individual to enhance their personal responsibility and autonomy

• Provision of support and facilities for the individual’s participation and integration into the community

• A standardized system of remote support, through which the individual can report on the evolution of their health and well-being
