One View, a predictive and preventative homelessness system

Predictive system that allows social services to anticipate, prevent and reduce homelessness in the territory.

The One View system analyses the data set related to services for adults, children and homeless people. The analytical model includes data on income, benefits and schools, and obtains a comprehensive overview of citizens and households in the territory in order to identify existing risk situations that may lead to homelessness in the near future.

One View enables this preventive approach by using advanced analytical models to identify people at risk, with the aim of detecting potential problems 6 to 9 months before a crisis. In addition, the service provides long-term monitoring, so that social services can also evaluate the effectiveness of the interventions put in place, and adjust them. The specialised software provides robust information governance and security, with strict protocols that determine access levels.

The service enables the district to achieve three objectives. The first is to improve the quality of life of residents by providing the necessary contact and assistance when needed. Secondly, a prevention of evolving needs with better informed and targeted interventions. In other words, social service professionals in the district have a unique view that allows them to select the most appropriate and effective course of action in each case. Finally, it contributes to the management of the demand for social and housing services, generating savings due to the combination of improved interventions and predictive information to identify at-risk situations.

A set of multidimensional geriatric assessment instruments designed to determine the psychosocial and functional capacity and needs of the elderly. The aim is to develop a comprehensive treatment and long-term follow-up plan based on a set of standardized items that can be used in various care settings.

InterRai works in such a way that social and health professionals talk to users and their families (people over 65 years of age), while making observations and using other information. The assessor codes this information with software that creates a picture of the person’s functional, social and health needs and opportunities, as well as individualized and specialized care support that is included in an anonymous database. Data collected from the population can be used for planning, research and service development.

Outcomes include validated scales and measures, as well as action points that trigger social, health and potential rehabilitation problems. Subsequently, each person can be followed up and their progress monitored. In addition, a complete profile of each user is created, minimizing the assessment burden and ensuring an individual perspective.

The tool is committed to improving health and social care around the world to ensure affordable, quality access to all people. When data becomes the core of understanding the care needs of older people, digital tools and proven solutions that can be applied on a large scale to help older people, caregivers and policy makers are needed to be used.

DoppelPlus, engaging low-income households in the fight against climate change

Project that helps low-income households become ambassadors of the transition to clean energy and promotes changes in the daily behavior of households in relation to climate change, a group particularly vulnerable to its consequences.

The initiative consists of sending starter kits to low-income families to optimize their daily consumption. In this way, they save money and also contribute to the energy transition. The aim is to reduce energy costs while improving their financial situation and quality of life.

The project is implemented by means of personalized guidance, whereby families receive advice on how to protect the climate, as well as tailored measures such as training programs or teaching the basics of climate protection. The initiative offers multiplier measures for climate protection and increased energy efficiency in areas such as housing, heating, electricity, mobility, consumption and food.

DoppelPlus has also trained 1,000 families in the Tyrol (Austria) to become ambassadors of the clean energy transition, with annual savings of €200 per household. Long-term partnerships have also been forged with energy suppliers, cultural organizations, social housing companies and an extensive communication campaign to raise awareness of energy issues. The project creates a win-win situation for everyone: the environment and the household budget.

School of training and dynamization of anti-loneliness agents, people over 55 years of age who, thanks to a qualification and accompaniment, specialize in early detection and preventive action in relation to loneliness.

The initiative refers to unwanted loneliness, i.e. the subjective feeling or experience that is identified with individual dissatisfaction that the person has in terms of relationships. This social isolation, especially related to the elderly, is perceived as a limitation, lack or absence of primary or natural relationships; friendship, partner, family, neighborhood, etc.

The project offers training and awareness-raising activities for people interested in the subject, whether they want to acquire knowledge in this area or are part of organizations, companies or associations. In this case, they can become part of the Agents Network, the other main activity of the project. This network, formed by people over 55 years of age who are not in a situation of loneliness and want to commit themselves and create proposals for action, consists of identifying situations of loneliness, design and creation of prevention projects, in order to accumulate evidence and be able to be implemented.

In addition, the initiative also has a repository of knowledge and good practices, where projects, methods or systems that involve the community and relational spheres can be found. The SSI Group understands that the connection between initiatives is a key element to guarantee the success of the projects. Therefore, Bizkaia Saretu is a project with a preventive perspective, which promotes a destigmatizing vision of loneliness, promotes mutual help and focuses on the individual needs and demands of each person.

Remote service to guide unemployed people in their job search, aimed at resolving doubts and accompanying them in the employability process through different channels and online activities.

Its objective is to provide job seekers with tools and information through a free telephone line, email, a Whatsapp channel and social networks (Instagram, Facebook and Twitter). With a call or message, the service’s professionals provide a personalized response in less than 72 hours to the questions and queries raised by users.

In addition, the CRE-e multichannel has a content platform accessible to everyone free of charge. There are two main tools; the first is online conferences that aim to help people improve their employability. Each week they invite expert speakers to talk about various topics: employment and training resources, job search tools, labor market, and possible market niches. They also discuss specific initiatives aimed at young people, tips to give visibility to the curriculum for people over 50, and tools to work on digital skills, among others. Secondly, within the web platform there are all kinds of key resources so that people can resolve doubts autonomously. In addition, specific content on women’s issues (gender violence, labor rights, etc.) is included.

CRE-e complements the personalized itineraries that have been developed in Red Cross since 2014 for people at risk of social exclusion, a comprehensive response to people who present diverse situations and trajectories, in order to alleviate the situation of vulnerability they live in.

The platform collects and manages data on children encountered in emergency migration contexts, and combines proven tools, global best practices and the latest open source technology to offer social service and humanitarian protection workers an easy-to-use and scalable solution to their data management challenges.

Primero enables its users to document events and violations to provide protection programs with up-to-date information on risk factors and patterns of violations. At the same time, child victims of violence can be linked to the services they need and prevention measures can be implemented to mitigate future risks. The platform’s powerful analytics and reporting tools ensure that the data is actionable.

Primero offers intuitive digital forms and clear workflows to help document case management processes, from identification and registration, to assessment, case planning, referrals and transfers, and case closure. With robust monitoring capabilities including email notifications for certain processes.

The platform uses sophisticated matching technology to match follow-up requests made by caregivers with the records of children registered as separated or unaccompanied. Matching criteria can be configured to meet the needs of the context. These functions are supported by a comprehensive case management module to ensure that children receive appropriate care.

Primero is a progressive web application (PWA) and leverages the latest technology to combine the best of web and mobile apps. The web app works across all browsers, regardless of operating system and on any device (mobile, tablet, laptop). It requires no previous user experience and can also be used offline, so that social workers can manage their work from anywhere, wherever they are. At the same time it offers the security and efficiency of digital case management in low connectivity configurations.

This service promotes the good treatment of elderly people in a situation of fragility and advises professionals who act in situations of abuse.

The SEAP (Specialised Service for the Care of People) bases its task on three lines of work. The first is prevention, through advice and training for professionals working with elderly people and people in situations of fragility. The second is social awareness-raising, thanks to talks, workshops, events and campaigns to promote collective awareness of the need to defend the good treatment of people. This area also includes specific actions aimed at professionals, citizens, adults, children and adolescents, the elderly and people with functional diversity. Finally, intervention, which is based on the coordination of actions with social services and other municipal services in situations of mistreatment of elderly people or people in situations of fragility.

The objectives of the SEAP are based on improving the quality of life of elderly people and people in situations of fragility, guaranteeing the prevention, detection and care of poor treatment and mistreatment through interdisciplinary work and networking with other services in the territory. It also responds to the real needs of the professionals who care for elderly people and people in situations of fragility when situations of poor treatment and/or mistreatment are detected.

It also aims to adapt the transmission of essential social, legal and medical information to the rest of the professionals in order to design a joint plan, to respond to the real situations of poor treatment and/or mistreatment of people in these groups and to guide and advise each situation individually, coordinating public and private resources to facilitate the necessary care.

An initiative of the International Organization for Migration (IOM) which, through a mobile app, offers a platform of relevant and up-to-date information so that migrants around the world can make informed decisions. It is available in 8 languages: English, Chinese, Arabic, French, Spanish, Italian, Portuguese and Russian.

Migrants from all five continents need a trusted source of information that also provides them with assistance and support to migrate safely. MigApp is an application that helps migrants to make informed decisions by providing information organized in various thematic and location areas.

In addition, the application also provides the possibility to make international bank transfers at no cost. It also has an option to find out the location of friends and family.

It also provides information on travel risks, access to IOM programmes and services for migrants, and data on migration profiles and trends around the world. This makes it useful not only for migrants and their families, but also for NGOs and governments.

Big data system for the comprehensive monitoring of cases of gender-based violence that brings together and coordinates the different public institutions in Spain with competencies in the area of gender-based violence. In this way, the system integrates all the information of interest deemed necessary to be able to assess the existing risk and, depending on the danger involved in each case, the monitoring and protection mechanisms best suited to the needs of each victim are activated.

VioGén provides women victims of male violence with a “Personalized Safety Plan”, with self-protection measures. Likewise, preventive work is carried out by issuing “Automated Notifications” when an incident is detected that could endanger the integrity of the victim.

The aim of all this is to establish a network that allows for better monitoring and protection of the victims, in a rapid, comprehensive and effective manner, whether they are battered women or minors who depend on them.

The Barcelona City Council’s Directorate for Ageing and Cures promoted this project with the coordination of the Department for the Promotion of Older People and the Directorate for Social Innovation, to take an interest in the emotional well-being of people aged 70-84 who were not linked to municipal services in the context of the COVID-19 pandemic and to inform them, when necessary, about existing resources that could improve their emotional state and their social isolation and/or loneliness.

Throughout the entire project, between June 2020 and July 2021, 53,382 people aged 70-84 were called automatically via 227,153 automated calls. Of the people called, 5,433 responded that they did want someone representing the City Council to call them personally on the phone. As a result of this, 7,961 personal calls were made, in which a telephone conversation was held with 4,714 people, filling out a form of 16 diagnostic questions (about loneliness or social isolation, sadness, anxiety, worries, information needs, etc.) to 4,360 people.

Verbio, a conversational technology company, using artificial intelligence and natural language processing, made the automated phone calls. Fundación Avismón, with expertise and sensitivity in the care and support of elderly people, was in charge of making the personal phone calls and providing information and guidance. And the Fundación Salud y Persona attended to 168 people expressing emotional distress, with professional psychologists by telephone (approximately 3 calls per person referred).
